What does this term mean?

We were talking about someone with an "issue" and the person said "It's manifested in him, directed at you". What does manifested in mean? Does that mean the "him" in our conversation is the one with the "issue" and he's directing it at me, or is it the other way around and that person was saying I'M the one with the "issue"?
