Does anyone know what tax form to file for military member who has rec'd enlistment bonus?

Hello, I was wondering if anyone knows what type of tax form 1040ez, 1040 or 1040 A, to file for military member that has rec'vd a enlistment bonus for early retirment. The total annual wages in box 1 rec'v was approx $70,000 dollars which included ythe bonus. Taxes were taken out before it was rec'vd of 7500.00. We did not rec'v any thing else besides the w-2 form, is there any other forms we should rec'v.? I always usually do the taxes on my own with 1040 ez, however if I use 1040ez this time I come up with a very large refund amount in which I don't believe could ever be right? just trying to do taxes myself if no luck with online help will get professional. thanks if anyone knows or can help.
